For issuing of certificates of origin Form A, its necessary to submit fulfilled certificate Form A, and statement of the producer signed by the authorized person. On the very certificate, on the back of the first and second page is explained how you should fulfill the certificate, especially what should be written in box 8. If it concerns a finishing process, then percentage of the finishing process is put and invoice of the imported material.

- If it concerns a finishing process when requesting certificate of origin of good “Origin”, the same documents are submitted plus a copy of the invoice of the imported material.
